Alternate Question Types


What are NCLEX-RN® Exam alternate questions?

Candidates may see questions on the NCLEX-RN® exam other than the traditional four-option multiple-choice questions. These alternate question types include multiple-choice items that require a candidate to select more than one response, fill-in-the-blank items, or pictures or graphs that require the candidate to identify a specific area. Multiple-choice items may also include charts, tables or graphic images.

What do these questions look like?

Hot Spots — Click on the correct area with your mouse pointer

The nurse performs a physical assessment on an adult male. Identify the area where the nurse should place the stethoscope to auscultate heart sounds heard in the tricuspid area.

Fill in the Blanks — Enter the answer

The nurse cares for a client receiving hourly peritoneal dialysis exchanges. During a one-hour exchange, the nurse infuses 2,000 ml of dialysate and 1900 ml. of outflow is returned. During the exchange, the client drinks 8 oz. of apple juice, 2 cups of water, and voids 150 cc of urine. Calculate and record the client's intake in milliliters.

The vast majority of the questions you see on the NCLEX-RN® exam will be multiple-choice, four-option questions.

Alternate questions have been added to the NCLEX-RN® exam pool of questions.

Will the alternate questions count?
According to the National Council of State Boards of Nursing (NCSBN), the alternate questions may count towards your results, or could be experimental questions being tried out for future exams.

Assume that these questions count toward your NCLEX-RN® exam results and answer them thoughtfully.
